Mr. Takao Yoko, Department of Neurosurgery, Tokyo Cixi Medical University, introduced the process of remote diagnosis of acute cerebrovascular disorders using the iPhone and iPad, highlighting the increasingly important role of modern mobile devices in adjuvant therapy. Effective use of critically ill patients "time before treatment" At present, in the field of cerebrovascular disease treatment, people pay great attention to "time before treatment." That is, the diagnosis and treatment of cerebral infarction due to vascular occlusion, the most important time period is the time before treatment. If the disease occurs within 3 hours, the use of rt-PA thrombolytic agent can effectively improve the cure rate. If it is within 8 hours, the thrombus that causes cerebral infarction can be removed from the blood vessels using the approved thrombectomy device Merci Retriever in 2010. The thrombectomy device has been clinically used in Europe and the United States since 2003. In recent years, clinical studies have reported its effectiveness. The National Association of Cerebrovascular Disorders carries out a call for action with the slogan "TIME LOST IS BRAIN LOST". The slogan means that the more time is wasted before treatment, the longer the brain function is lost. However, due to the medical system in Japan, few hospitals in Japan have cerebrovascular experts on duty 24 hours a day. Moreover, it takes a lot of time to transport from an ambulance to a hospital before treatment. In short, there are still many problems in the emergency medical system. Mobile devices give people access to information anytime, anywhere In the treatment of cerebrovascular diseases, the patient's receiving system and the correct early diagnosis are crucial. Therefore, our university has undertaken a project to develop a new remote diagnostic treatment support system. Launched in October 2009, the project aims to build a new medical information system based on the concept of “access to patient information at any location within and outside the hospital and the ability to seek expert diagnostic advice through the systemâ€. First of all, it is to use the mobile phone information terminal that has developed rapidly in recent years. We will use the information “i-Stroke†as the first target to receive the information “Whether, no matter where, when and whereâ€. Mobile) application software for development (Figure 1). Strive to realize the image needed in an emergency when necessary, through a high-security virtual private network (VPN: Virtual Private Network), so that each mode (CT, MRI or DSA, etc.) can be read simply and quickly (Figure 2). In addition, there is a system that is free to build and view 3D images – something that has not been available so far. In order to realize these functions, we worked with a Japanese company to develop a PACS (Picture Archiving and Communication System) with the goal of quickly forwarding and viewing images regardless of the circumstances. This is an important function in the field of cerebrovascular disease treatment, for the full support of the medical team, which can shorten the time for calling the whole staff. In the function of the system, it is also possible to display the time after the hospital and the passage of time, so that real-time understanding of the examinations performed on the patient and the image results (displaying the timeline timeline) at a certain point in time can be used to treat the brain. Relevant personnel of patients with vascular disease can share the same information and respond quickly. Application of mobile devices in interconnection According to our experience, by introducing the remote diagnostic and treatment system i-Stroke, even if there is only one doctor on duty, it is possible to ask for the diagnosis of each doctor and discuss the treatment method. This has extraordinary significance. In the treatment of cerebrovascular diseases, there are many alternative treatment methods. Compared with the judgment of the doctor alone, the experts and the superior doctors can jointly perform the treatment diagnosis, and the medical safety is higher. In this case, each expert and the superior doctor can simultaneously view the image on hand at the same time, and get the image information "unreachable by the language". Moreover, medical information is sent at the same time, and it can be judged even outside the hospital. It is expected that the future can be applied to the interconnection between hospitals, for example, according to the images of other hospitals for treatment discussion, sending patient introduction letters or saving patient information (blood sputum, past medical history, medication history, etc.) on the patient's mobile phone (Fig. 3) : Figure 3 Patient mobile phone application can display hospital check appointment items and outpatient appointment waiting time, etc. With the remote diagnostic and therapeutic aid system we developed, the diagnosis and treatment of patients can be carried out quickly. Even if it is rare, it can increase the chances of people who can be treated. Even one person can save the lives of many patients, and hope that this system will help the development of cerebrovascular disease medical care. By using a remote diagnostic treatment aid system, it can help to improve misdiagnosis, as well as current doctors evading responsibility (letting patients refer). Even if more patients with cerebrovascular disease are treated, the medical expenses of the country will be alleviated, and it is hoped that the medical industry will develop in a good direction.
